Grace House - opening soon! Family information sessions starting from April 2015
We will offer short break care to children and young people aged 5-18 who have a complex physical disability, health need and life limiting condition, living in the North East.
Usually you will be referred to Grace House by a professional who is already involved in your child's care. This usually follows a social work or health assessment, but the process can vary depending which Local Authority area you live in. Please talk to us about your individual circumstances and needs and we will do our best to help you through the referral process.
